Applicant's Statement

I certify that the information and statements contained in this application are true and complete to the best of my knowledge. I understand that any misstatement or material omission from this application may result in my disqualification from consideration for a position and may be a basis for the termination of my services. I understand that submitting this application form does not automatically register me as a volunteer tutor, but that there is a selection process including an interview and mandatory training. I confirm that I am willing to volunteer as a tutor for at least a six (6) month period.

I authorize Deerbrook Covenant Church or its agents to undertake any investigation it deems appropriate in connection with this application, including a criminal background check. I authorize any references listed to provide any information (including opinions) that they may have regarding my character and fitness to work with children or youth. I release any individual or organization providing information from liability. I waive any right to inspect any information provided about me by any person or organization identified by me in this application.
